Although most Non-Governmental Organizations and Non-Profit Organizations are usually started with a good plan and proper intentions, not to mention by individuals who are, or have been, very successful in other professions or ventures, it is an unfortunate fact that 30,000 to 60,000 NPOs will fail and disappear from the IRS's files each year.  This happens for a number of reasons, which include not filing necessary paperwork, inefficient marketing practices, financial issues, and the list goes on.  What is even more unfortunate is that as these organizations fail to reach their goals, the need for their causes is usually increasing as time goes by.
